My wife has an older Win 101 that has been tricked out to a trap gun. She shoots it very well. However she keeps getting misfires on the under barrel, first barrel to fire. It seems the hammer didn't cock. Ejectors on both barrels work fine when the barrels fire. I have watched and coached her to ensure she is opening and closing the gun to it's full cycle but occasionally it repeats. When I use the gun I have never encountered this problem. I am perplexed at what is the issue. I would appreciate some varied thoughts.

First thought? Strip and clean. Sonic cleaning is simple.
Make sure she opens the gun full stop. It if repeats, it's a worn part.

It could very well be that the hammer is not recocking. I had a BSS that did the same thing. Took it to Jim Kelly and he fixed it. Never has happened again, and I'm still shooting the same gun many years later at ducks and clays.
Probably not something you want to tackle yourself, as there are two or three things that can cause it, and experience fixing doubles is the best teacher as to how to best fix it.

Trigger return spring?
Her body and trigger finger likely move differently under recoil than do yours.
Pull stock, spray gunk out, dry, lightly lube all the pins and axles.
Springs do get tired.

had the same thing on my 1937 mod 90 twenty. it was suggested I peen out the cocking rods as the ends were a tiny bit mushroomed. worked perfectly every time after that.
not sure if this will help you....

Thank you. I asked for some varied thoughts and I am receiving them. A good number of remedies to follow up on that I certainly hadn't thought of. Number one is puling the stock and giving it a good cleaning. Something that escaped me given it is only shot in the cleanest and driest conditions.

I sure would like to know what actually works in the end. A friend has a Chas Daly side by 20 which will often not fire the first barrel for him but always does for everyone else. No explanation. Cleaning, among other things, had no effect. Sort of haunted I guess.

I had the same problem with my Winchester 96 12 gauge O/U. Remove the butt stock and open the action slowly; the ejector for the bottom barrel will kick before the action is opened far enough to cock the bottom barrel. I was easing my gun open to catch the empty shell at the trap field and frequently (about 20% of the time) had subsequent FTF of the lower barrel. Now I deliberately open my gun fully and it only occurs about once in 6 or 7 rounds of trap. Winchester 96 and (original) 101 were made by Olin-Koshenda in Japan.
