Ed: I'm sure you're aware of the "Darwin Award", if not...it's an award given to anyone so stupid they did themselves in before procreating, sparing the rest of us the trouble of dealing with their equally foolish and stupid offspring.
Do foolishly ignorant people sometimes use inappropriate shells in older guns and do those guns occasionally fail? The answer to both of those questions is clearly "yes".
Sadly...even fluid steel won't save most of these folks. To answer your earlier question of "which gun" I would respond with "any gun that requires lighter and more low-pressure shells". Most of these are my various "game" guns which are as light as I can find them, but... I must admit that I do also use heavier loads in my target guns, some of which are even "Twist" steel.

You might note the thickness of the tubes on breech-end of this one. That "thickness" continues throughout the barrels.
Modern ammo means nothing to it.
