A jeweler or silversmith can make one for you. First a tracing of the existing image is made then transfered to sheet silver. Next the shape is cut out with a jewellers saw then tiny nails are soldered to the back. Most likely that is the process that made the original. On the other hand the original may have been sculpted in wax then cast in the lost wax method. A skilled jeweler or silversmith can also make one that way.
