Hump; I think the way it generally happens is they are otherwise distracted in some manner, as just having unloaded the dogs & talking with their partner etc & pull a couple of shells & drop them in, close the gun without looking. Sometime later they get a shot & the gun snaps. Upon opening it the glance at the breech & seeing no sheel "Kick Themselves" for forgetting to load the gun & losing a bird & procede to drop two more atop. Several glaring mistakes, but has happened & likely will happen again. Has also happened on target ranges, again loading with their thoughts somehow preoccupied & again think they forgot to load.
