I received an email from Mark DeHaan after sending him the photo and he said that was normal on a new gun and would correct itself after much use.
With the barrels removed the lever does return to dead center and the Greener bolt sits flush. The gun is an SO not an SC and was their entry level gun in 2005. He also said that it a shotgun has a perfectly centered lever it meant it was well used.

He sure is. I am considering a custom 16 gauge from him and have asked him dozens of questions and he has replied to each one personally.
