Loosely held guns will often not reset to the 2nd trigger particularly with light loads. And of course - clean and lube properly

So I fully understand, the gun is loaded, and when she pulls the trigger, nothing happens? Or when she pulls the trigger, you hear a snap and the gun does not fire? One thing to look at, is the opening lever being closed all the way? Sometimes with ammo, dirt, lack of shutting smartly, even lint or clothing getting in the way, the lever does not fully close and will then evidence itself in not having the lower barrel fire. Clean the gun and go from there. Most times that's the only repair needed on these. As noted you do need to make sure they are fully being opened to cock them. Ruger Red Labels have the same problem.

Check that the safety is moved into the proper position, either upper or lower position when it is pushed off. On some guns, you can easily get the safety selector in between the positions and when pushed forward, it cannot get to either sear. funny you don't have any problems.

I have double checked the safety I don't think that is the problem. The slide moves well into its proper position.
Nothing is foolproof to a sufficiently talented fool.
